Landlords
Landlords interested in participating in our HCV program can fill out the following landlord application forms and submit them to us, and be on file as a potential landlord. Submit the completed forms to [email protected]. - Form W-9 - Direct Deposit Form - Landlord Participation Agreement If you would like to renew your lease with a tenant and/or update the rent amount, please fill out and submit the Lease Renewal Agreement form.
